Abstract

Background: Tuberculosis and diabetes mellitus (TB-DM) comorbidity is a health problem that can worsen a patient's condition and increase the risk of complications. In Indonesia, the prevalence of TB-DM comorbidity reached 42.6%. In 2025, Kuningan Regency recorded 131 cases of TB-DM comorbidity, while in Ciawigebang District, 25 cases of TB-DM comorbidity were found between January 2025 and March 2026. Several factors such as nutritional status, blood pressure, smoking habits, and history of TB contact are suspected to be associated with the incidence of TB-DM comorbidity. This study aims to determine the factors influencing the comorbidity of Tuberculosis and Diabetes Mellitus in Ciawigebang District, Kuningan Regency in 2026. Methods: This was an observational analytical study with a case-control design. The cases were patients with comorbid TB and DM, while the controls were community members without TB and DM. The sample consisted of 75 respondents, consisting of 25 cases and 50 controls (1:2). Sampling was conducted using total sampling in the case group and purposive sampling in the control group. Data were analyzed univariately and bivariately using the Chi-square test. Result: The results showed that the case group had abnormal nutritional status 84%, complaints of hypertension 84%, smoking habit 60%, and history of contact with TB 72%. Bivariate analysis showed a significant relationship between nutritional status (p=0,000; 0R=14,942; 95%CI=4,315-51,739), blood pressure (p=0,000; OR=13,500; 95%CI=3,928-46,402), smoking habits (p=0,000; 13,500; 95%CI=3,977-45,830), and a history of TB contact (p=0,000; OR=61,714; 95%CI=11,708-325,294) with the incidence of TB-DM comorbidities. Conclusion: There were significant relationships between nutritional status, blood pressure, smoking habits, and history of TB contact with the incidence of tuberculosis and diabetes mellitus comorbidity in Ciawigebang District, Kuningan Regency, in 2026. Therefore, promotive and preventive efforts are needed through health education, risk factor control, and TB transmission prevention to reduce the incidence of TB-DM comorbidity.
