Abstract
Background: A high nurse turnover rate can disrupt operational stability and the quality of healthcare services in hospitals. Implementing a structured career ladder system is an essential effort to enhance professionalism and reduce nurse turnover intention. This study aims to analyze the correlation between nurses' career ladder and turnover intention in the Inpatient Unit of RSUD 45 Kuningan in 2026. Methods: This quantitative study utilized a correlational design with a crosssectional approach. The sample consisted of 62 inpatient nurses selected using a total sampling technique. Research instruments included observation sheets and interviews for the career ladder (Clinical Nurse I–V) as well as a tested questionnaire (Cronbach's α = 0.964) for turnover intention. Data were analyzed univariately and bivariately using the Spearman's Rank correlation test. Results: Most nurses were at the Clinical Nurse II (PK II) level, accounting for 24 respondents (38.7%), and PK III level with 22 respondents (35.5%). The overview of nurse turnover intention was mostly in the high category, involving 36 respondents (58.1%). Bivariate analysis showed a significant relationship between nurses' career ladder and turnover intention (p-value = 0.035 < 0.05). The correlation coefficient value of r = 0.269 indicates a weak positive relationship. Conclusion: There is a significant but weak relationship between nurses' career ladder and turnover intention at RSUD 45 Kuningan. Hospital management is advised to improve the transparency of promotion pathways and provide proportional incentives and rewards to reduce nurse turnover intention.
